A SOCIAL SECURITY NUMBER IS REQUIRIED FOR EVERY PATIENT AND INSURANCE POLICY HOLDER. APPOINTMENTS WILL NOT BE SCHEDULED WITHOUT PROVIDING THE PATIENT/POLICY HOLDERS SOCIAL SECURITY NUMBER.
Please be sure to bring your social security number, insurance card, Driver’s License/ ID to EVERY visit.
